Federal OSHA recorded a severe workplace injury at Agrex Inc., 11th ST Pier D, MOBILE, ALABAMA 36602 on — Fractures, affecting the pelvis.

Three employees were changing a fan motor on a platform. The employees removed the old motor and lowered it through an opening in the catwalk/platform that was approximately 4' x 4' using a chain hoist. They then hoisted up the new motor and were securing it into place. Employee 2 called employee 1 to look at the alignment of a series of pulleys to make sure it was correct. When Employee 1 stepped around the motor, they stepped into the opening in the platform. Employee 1 grabbed onto Employee 2 pulling him into the hole in the platform as well. Both employees fell approximately 12 feet to the platform below and employee 1 struck the old motor that had been removed. Both employees suffered broken bones, contusions, and abrasions. Employee 1 was hospitalized for a broken pelvis.

K&R Mechanical Services, L.L.C.

An employee was checking an HVAC unit in the attic when they fell approximately 15 feet through the rafters to the floor. The employee sustained fractures to the ribs and scapula as well as a collapsed lung.

Superior Forge and Steel Corporation

An employee was removing grating to change metal chip hoppers located in a pit. The employee lost his balance and fell approximately 12 feet from the grate, landing in the metal chip hopper. The employee sustained a sprained right foot, a left foot fracture, and a fractured L5 vertebra.

General Fire & Safety

An employee was reviewing the next steps to take with the foreman when some material fell from above and landed near them. The injured employee then looked up, took a step on the catwalk, and fell through a gap, landing on the ground 15 feet below. The employee sustained three fractured vertebrae and a head laceration.

0n November 16, 2023, an employee was climbing onto a catwalk to cut valves for unit heaters in a building. The employee fell 14 feet through the attic onto the concrete floor, resulting in a fractured pelvis.

PATTEN CONSTRUCTION, INC.

An employee was removing a rooftop curb when they fell approximately 15 feet through an opening in the roof, resulting in a fractured leg that required hospitalization.

Central Industries, Inc.

An employee was working on removing the gearbox from the drive shaft of the motor assembly of a feather drier. He was using a hydraulic tool to pull the pieces apart when a bolt dislodged from the eye of the gearbox and the corner of the gearbox broke off. The employee was struck in the nose (sinus cavity), resulting in hospitalization and surgery.

An employee was on a forklift checking the sprinkler system drain. The employee fell 8 feet from the forklift to the floor. The employee suffered a broken collar bone and was hospitalized.

G&H Pets LLC

An employee was working as a dumper on a packaging line. A passing forklift turned and pinned the employee against a guardrail. The employee suffered fractures to their ribs and a leg requiring hospitalization.

Gorman Milling Co. Inc.

An employee was cleaning an upper mezzanine. When he leaned against the mezzanine gate, he fell through the gate opening and landed 11 feet below. The employee sustained fractured orbital bones.

Sanderson Farms LLC, Production Division

An employee was assigned to clean a feed mill in the basement. He then rode a tool and material basket to return from the basement to the ground level. He fell out of the basket and landed about 5 feet below. The employee sustained fractures to his arm, lower back, and foot.

Sysco Gulf Coast, LLC

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

Norflex, Inc.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
