Final narrative
On February 8, 2022, at 9:20 a.m., an employee was slicing chicken utilizing a slicer. She went to remove a small piece of chicken breast that was lodged between the blade and the slicer tray when the blade contacted her left index fingertip, resulting in amputation.
In the kitchen prep area, an employee was operating a meat slicer to cut chicken breast into strips. The employee went to remove a piece of chicken that was stuck between the meat grip and the bottom of the slicer tray. Their right thumb got caught underneath the meat grip and the blade of the slicer amputated their thumb tip.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.
