On March 15, 2022, an employee slipped while climbing down a truck ladder. The employee fell to the gravel about 7 feet below and suffered a compound fracture to the ankle.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

Absolics Inc.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was riding a horse while waiting to start sorting cattle when the horse started bucking. The horse's head hit the employee's jaw and the employee fell off the horse, resulting in a broken jaw.

An employee was opening a gate to pen a bull. The bull started fighting with another bull and drove the other bull into the employee, which drove her into the fence and knocked her to the ground. The employee sustained a fractured sternum, four fractured ribs, a lacerated spleen, a fractured L2 vertebra, and a tear to the posterior cruciate ligament (PCL) of the right knee.

An employee was operating a vertical mast skid steer loader as he was moving bags of corn from the calf barn to a collection site. The employee unloaded the bags of corn when the attached forks, which were extended up, detached from the loader. As the forks swung downward, the force caused the forks to crash through the operator's front cab door and into the employee's legs. The employee sustained two broken legs and was hospitalized.

New Holland Sales Stables

An employee was loading a bull into a chute for tagging. The gate did not latch, and the bull knocked the employee down, then threw him up into the air. He suffered broken ribs.

An employee was latching the gate after penning a bull when the bull hit the gate and ran over the employee's legs, resulting in a fractured left leg.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
