He used a pair of snips to cut the nylon banding off the top bundle. The top bundle moved and the bottom board fell to the ground. As the employee moved away from the stacked bundles, he tripped over the forks and fell to the concrete ground. The employee was hospitalized with fractures to the right ilium and femur.

An employee was assisting on a refiner in a paperboard mill. While his hand was in a drain, a valve was opened and hot water splashed onto and severely burned his right hand and wrist.

While closing the valve, pulp stock entered the top of his boots and caused second-degree burns to both of his lower legs down to his feet. The employee was hospitalized.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
