An employee was running a blow molding machine and blowing gallon jugs when his right hand was pinched in the machine. He sustained amputation of the ring fingertip and fractures to the middle and little fingers. The machine was guarded at the time.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was walking between two production lines when he tripped and fell over a pallet that had some equipment on it. The employee fell on his right side, resulting in a fractured hip and a possible thumb injury.

An employee was using a stand-up forklift to load a small box truck. As he was putting down a pallet to push it onto a truck, the weight caused the truck to move forward. This pulled the forklift forward and it dropped between the truck and the loading dock, causing partial amputations to their left little and ring fingers.

An employee was unloading product from a trailer onto a scissor lift. They stepped backward and fell approximately 5 feet from the scissor lift to the concrete floor, sustaining six broken ribs and a punctured lung.

His right ring finger got pulled under the metal product guide rail at the edge of the conveyor and was pinched between the conveyor and the guide rail. The employee sustained an amputation of the fingertip between the first knuckle and the fingernail.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.
