An employee was polishing a part on a manual lathe. The lathe injured the employee's hand and forearm.

Dril-Quip, Inc.

An employee was using a crane to move a 6,000-pound piece of equipment into another piece of equipment. The first piece did not fully go inside the second, causing the nylon lifting straps to loosen and making adjustments necessary. As the employee made adjustments, the 6,000-pound piece suddenly dropped into the other piece; the nylon straps tightened on the lifting bail ring and crushed the employee's fingers.

An employee was attempting to get lifting straps underneath a 2,100-pound steel part when the part came off the straps and crushed two of his left-hand fingers.

A mechanic was at machine 4L-2 in the rough machine center when a machinist asked to borrow the 10-ton overhead gantry crane. As the machinist moved the crane, the hook struck the mechanic in the head. Later, the employee had a headache and blurry vision. He was taken to the hospital and admitted.

An employee was fitting an arbor fixture into a rotor housing to be reworked. The rotor is moved by a hoist. The arbor was being lowered into the rotor housing when the arbor slipped down onto his hand and amputated a portion of his left thumb.

A crew was tripping out of the hole with an electrical submersible pump (ESP) in the process of swapping tubing tong heads to break down the ESP. An employee's left arm was caught and pinched by the tubing tongs. The employee sustained three lacerations and a fracture to the arm.

On May 21, 2025, at approximately 3:40 PM, a machinist was preparing a CNC lathe machine for it's next run. He was closing the machine door to complete the tooling setup when the door crushed his right middle finger. The employee required a surgical amputation to his fingertip.
