Two employees were working from a catwalk about 7 feet off the ground. The catwalk structure pulled out from the wall. It and one of the employees fell to the ground. The catwalk members pinned the employee's legs, injuring both (in particular, causing a dislocation to the left knee with blood vessel impairment).

An employee was removing a safety railing from a stairwell. The 2x10 plank they were standing on gave way and the employee fell approximately 12 feet to a lower level, resulting in fractures.

An employee was climbing a ladder to get to the top of a container. The ladder collapsed and the employee fell 13 feet to the concrete floor, sustaining a fractured pelvis and tailbone.

An employee was on a roof when the material under his feet gave way. He fell 12 feet, suffering an injury to back vertebrae and a dislocated shoulder.

Two employees were cleaning the ducts on top of a pipe. The pipe gave way and the employees and the pipe fell 15 feet. The pipe landed on one employee's leg, resulting in a fracture in three places and hospitalization. The other employee was not injured.

An employee was walking on the roof deck performing roofing work when the deck collapsed. The employee fell approximately 13 feet to the ground below, resulting in fractures and bruising to the hips, head, and chest.

On September 16, 2025, an employee was installing a billboard advertising display when they fell 8 feet and 6 inches to the ground. The employee was hospitalized with a puncture wound.

An employee was installing a piece of advertising copy on a billboard. He fell approximately 18-25 feet from the structure to the ground, suffering broken ribs and facial trauma.

An employee was installing a sign. They left the bucket to reach bolts, removed their harness, and reached for a piece of iron. The employee climbed to the back of the sign on the affixed metal lattice. The bolt came loose and the iron gave way, causing the employee to fall approximately 13 feet to the ground. The employee sustained fractures to their hips, arm, and shoulders. They also sustained torn rotator cuffs in both shoulders. The employee was hospitalized and required surgery.

An employee was climbing a hook ladder. A piece of advertising material fell onto the employee, causing them to slip and fall from the fourth or fifth rung. The employee landed on their left arm, breaking the arm and also suffering a broken rib.

An employee set up and secured an extension ladder on a pole so he could tighten up a low-hanging triplex cable. While on the pole, the employee started to pull the triplex cable to raise it up. The tension caused the pole to snap. The employee jumped off the ladder to avoid getting hit by the pole. The employee impacted the ground and sustained bilateral talus fractures.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
