A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

A patient kicked a police officer in the leg, rupturing their patellar tendon. The police officer required hospitalization and surgery.

A cashier was hit in the face by a customer. The employee sustained injuries to his head and face.

An employee was making patient rounds at the hospital and stopped to talk with the charge nurse in front of the nurse's station. A patient reached over the nurse's station, grabbed a metal three-hole paper punch from over the desk, and proceeded to attack the employee with it. The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was leaving the building when they tripped on the steps and fell to the ground. The employee sustained a fractured left foot and a sprained right ankle.

An employee was attempting to restrain a resident when their feet got tangled and both fell to the floor. The resident fell on top of the employee who struck a corner of the wall and then the floor, resulting in a broken right hip.

An employee was descending stairs when they lost balance and fell down 3-4 steps to the second floor landing. The employee was hospitalized with two broken ribs and a concussion.

An employee was assisting a client with walking when they tripped over a kitchen rug and fell. The employee sustained a nose laceration and a swollen eye. The employee then lost consciousness and had seizures, requiring hospitalization.

An employee was stepping from a gravel parking lot onto a cement-paved sidewalk. She tripped and fell, suffering a concussion, a broken nose, and injured vertebrae. She was hospitalized.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
