An employee sustained a partial amputation of the left index fingertip when his glove was caught in a cutter machine as he was removing a piece of chicken. The machine was not locked out/tagged out at the time.
On July 22, 2020, at approximately 2:00 AM, an employee was preparing to wash down equipment. When he opened the door of the equipment, his left thumb was pinched against the power panel attached to the opposite wall, causing an injury that required surgical amputation of the distal portion of the thumb.
