An employee cleaned a spill of chemicals including ester, amine, and amide. The employee then cleaned a spill of sodium sulfite which mixed in the disposal container with the chemicals from the previous spill. A chemical reaction resulted and the employee inhaled sulfur dioxide gas. The employee was hospitalized.
An employee was unloading a tanker trailer when he heard an audible leak from the nitrogen valve and shut it off. When he stepped onto the trailer to investigate, the wash cap (three inches wide) popped off and hit him in the face. He sustained fractures to his nose and orbital bone, an open skull fracture, and lacerations to his face.
