An employee was working on the brakes of a bicycle. As he spun the bicycle's wheel, the disk brake amputated the flesh from his fingertip.
An employee was using a ladder to retrieve a set of golf clubs from a shelf. While descending the ladder, the golf clubs slipped and struck him in the head, causing him to fall to the floor. The employee sustained a right hip and femur injury.
