An employee was changing the fruit feeder to a funnel adaptor piece when they took the impeller plate off and the gasket came off. When retrieving the gasket, the spinning pump impeller caught his finger resulting in an amputation of the right index fingertip to the first knuckle.

An employee was retrieving bread dough from a dough rounder when their right middle finger was struck by a dough blade, resulting in a fingertip amputation.

An employee was pushing a dry bunker rack of restaurant supplies onto a delivery truck when a shelf came down and pinched the top of the employee's finger, resulting in an amputation.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was using an end mill to mill out a section of a metal door frame that he was fabricating. Metal shavings built up and he could not see the marking to cut, so he used a small brush to remove the shavings. While brushing off the shavings, his glove became caught in the spinning mill bit and pulled his hand in. The employee's right index finger contacted the spinning bit, resulting in a partial amputation to the side of the finger.

An employee was unjamming cookie dough from a machine when the rotating pump caught her left index and ring fingers, resulting in fingertip amputations.

An employee was marking a malfunctioning mold when their finger got caught between one section of rollers on the mold and the guarding on the next section of rollers. The employee's finger was amputated below the nailbed.

An employee was exiting the rear door of a freezer truck when he slipped/misstepped and fell to the ground. The employee sustained a broken right leg that required surgery.
