Employees were standing by an agricultural tractor with a chemical applicator to see if the installed fan was working properly. The tractor's RPM shot up and the fan began to spin faster than normal causing the fan's protective casing to explode and shoot metal pieces towards two employees on the opposite side of the tractor.
