Final narrative
On July 2, at 3:50 PM, an employee got out of their car and was walking in the driveway. The employee dropped their keys on the driveway. While picking up the keys, the employee lost balance and fell onto the concrete driveway surface. The employee was hospitalized with a compression fracture in their lower back, a jammed finger, and pain in their right elbow and wrist.
On July 28, 2019, at approximately 2:15 p.m., an employee was in the office when her knee gave out. She fell to the ground and sustained fractures to the left shoulder and was hospitalized.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
