An employee was using a manlift (order picker) to collect items for delivery. They were raising the work platform when their left middle finger was crushed between the overhead cage of the lift and the ceiling trusses. The employee's fingertip was amputated.

An employee was on a rolling staircase ladder, retrieving a box of nails. He fell from the ladder and his head struck the floor. He suffered a suspected brain bleed.

An employee was stacking a load of drywall using a forklift. As he adjusted the load, his right index finger was caught between two pieces of sheetrock and was lacerated to the bone. The employee was hospitalized and required surgery.

An employee was sitting on a stool at a desk. As they went to stand up, they tripped over the bottom of the stool and fell. The employee sustained a hip fracture that required hospitalization and surgery.

An employee was in a parking lot, trying to separate a flat cart from a regular shopping cart, when he fell. He suffered a broken hip and was hospitalized.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

The injured employee was walking through the warehouse. Another associate was driving a forklift with a pallet of large format tile (slabs) when the slabs began to tip. The injured employee ran over to stop the slabs from falling, but the slabs fell on his lower left leg/foot. The employee sustained fractures in his lower leg and foot. He was hospitalized and required surgery.

An employee was doing a cutback on a piece of lumber using a chop saw. The saw hit a knot, the piece of lumber buckled, then kicked back and broke the kickback guard. The board was then pulled back into the blade, which lacerated the employee's left index and middle fingers.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was descending a rolling ladder when she fell from the last step and her head struck the floor, resulting in a fractured pelvis.

On September 24, 2025, an employee was using a sledgehammer to straighten the forks on a forklift when a piece of metal broke off the fork and became lodged in the employee's leg. The employee was hospitalized.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
