Two employees were pulling a stock cart filled with product out of a meat cooler. One employee pushed the cart and the other employee's right arm was caught between the cart and an asset-protector bollard. He sustained a fractured upper right arm.

An employee was cleaning the produce production room. The employee was spraying a grease cutter chemical when it mixed with an antimicrobial agent in a jug under a sink. The injured employee was called over to assess the situation, picked up the jug of antimicrobial agent, and inhaled the vapors from the chemical mixture. The injured employee was hospitalized.

An employee was processing meat on a meat saw. His hand slipped and came into contact with a circular blade, which lacerated his right index finger, resulting in a partial amputation. The employee was hospitalized. The machine was not guarded at the time.

An employee was standing on the operator's platform of a powered pallet jack. During a turn, the employee fell to the warehouse floor and landed on his left hip, resulting in a fracture to his upper femur.

An employee was operating a meat saw and sustained a partial amputation of the right middle finger including the fingernail.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
