On 8/17/2022, at 2:25 PM, an employee was making a delivery to a customer. The employee was moving a two wheel manual handcart down from a tractor trailer on a grated metal ramp when they slipped and fell on the ramp. The employee attempted to brace his fall with his left hand and the left little fingertip was caught in the grating of the ramp and amputated.

An employee was pulling a handcart with product on it while unloading a trailer. While on the elevated liftgate, the cart ran over his foot, causing him to fall 4 feet from the liftgate to the ground. The employee sustained a rib and collarbone fracture, as well as a punctured lung.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

Two forklift drivers were stopped next to each other and were talking. The injured employee leaned out of their forklift as the other forklift was leaving and they became pinned between the two forklifts. The injured employee was hospitalized with a fractured rib and a lacerated lung.

A crew was preparing a 20-foot section of 12-inch high-density polyethylene (HDPE) pipe to be swung into a welder using an excavator, where it would be welded onto a longer run of pipe. One employee was facing the longer pipe in the welder while the 20-foot section was being rigged. The 20-foot section moved and struck the employee's left knee. The employee's left leg was broken below the knee.

Greggo & Ferrara, Inc.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

GREGGO AND FERRARA, INC.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.
