An employee pushed a sectional sofa to move it out of the way of other employees using a pallet jack. Rather than sliding, the sofa tipped over. The employee fell, striking the back of her head on a metal cabinet. She suffered a brain bleed.
An employee was on a two-step ladder performing cleaning activities in the de-oiler machine. The employee fell back and his right arm was caught in the equipment, resulting in a right forearm fracture that required surgery.
