Final narrative
A metal reel was dropped off the production line and an employee's right little fingertip was crushed between the reel and the flange/arm. The employee sustained a distal amputation to their right little finger.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
Employees were hooking up a new reel of pipe to be processed into shorter lengths. This process involved routing a winch cable through a haul-off and attaching it to the pipe using a clamp. One employee held the winch cable while another pressed the feed-out button to loosen the cable. The cable tightened and pulled the hand of the employee holding it into a pulley. At about 1:25 a.m. on August 14, 2025, an employee was operating a 10-inch band saw at a molding machine. The employee was working to cut the sprue off a PVC fitting. The blade snagged on the sprue and pulled his left index finger into the blade, which cut it. The finger was partially medically amputated.
An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
