The blade came down and amputated the employee's left index fingertip above the first knuckle and the tip of their left thumb.

An employee was adjusting a ring on a bar bending machine when their glove became caught in a pinch point on the machine. The machine was activated and the employee sustained fractures to the four fingers on their right hand and amputations to the index and little fingers.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.
