The material became jammed in the blade, which froze in the partway down position. The employee began working to unjam the material. The machine activated, causing the blade to become free. The blade came down and amputated the employee's left index fingertip above the first knuckle and the tip of their left thumb.

An employee was adjusting a ring on a bar bending machine when their glove became caught in a pinch point on the machine. The machine was activated and the employee sustained fractures to the four fingers on their right hand and amputations to the index and little fingers.
