An employee was using an automated press machine to remove the center label from a 12-inch vinyl record. As the employee pointed to something on the running press machine, the tip of his finger was amputated; it was later surgically reattached.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
