As she was tightening the safety rope that was tied around a bar that holds the gangway in place, the rope broke, causing her to slip and fall backward. She sustained a hematoma and laceration to the back of the head as well as a right elbow fracture.

A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

The employee put a 17-foot fiberglass stick against the pole, and was strapping a bungee cord around the pole so it wouldn t fall, when he contacted a faulty ground wire and sustained an electric shock.
