An employee was trying to unclog a pressurized oil supply line when a flange blew off the pipe and struck the employee, resulting in intestinal injuries.

FUJI VEGETABLE OIL, INC.

The employee was removing a hose from a tank of water/oil mix that was approximately 226 degrees Fahrenheit to use in another location. The valve was open when the hose was removed resulting in the hot liquid spraying out under pressure onto the employee. The employee suffered second and third degree burns to the face, neck, stomach, and both arms.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee had isolated, depressurized, and de-energized the south filter canister to replace a filter. They loosened the pressurized lid of the north canister, resulting in hot oil spraying them. The employee was hospitalized with burns to their arms and neck. Personal protective equipment (PPE) was not worn at the time.

An employee was unclogging product from a railcar hose using air pressure from a valve. The air hose coupling went out of control in the employee's right hand and struck the employee's right shin, resulting in a fractured tibia. The employee was hospitalized.

On March 22, 2023, an employee was using a broom to push down on a dock plate. The employee lost her balance and fell from the dock to the ground, sustaining fractures to the left leg, left knee, and right heel.

An employee filled a 55-gallon trash can with chemicals for cleaning and attempted to move the trash can when the liquid splashed. As the employee attempted to avoid the liquid, he tripped and liquid splashed onto him, resulting in chemical burns on the right side of his torso.

An employee was trying to hook a pallet that had stopped on the conveyor belt when the long hook tripped the sensor for the automatic shuttle car. The employee was pinched between the shuttle car and the racking system, resulting in surgery for lacerations on both legs.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
