Final narrative
At about 11:30 a.m. on November 8, 2022, an employee was using a meat saw to cut bone parts. The blade struck the bone, causing the meat to move. The employee's left thumb came in contact with the blade of the saw, amputating the tip.
An employee was picking up a stack of pallets with an electric jack. His hand was caught between the jack and the warehouse racking, causing a partial amputation of the left ring finger.
An employee was cutting a beef shank with a meat saw when a bone struck the blade causing the item to flip. The meat cutter blade caught the employee's right index finger and the employee sustained an amputation.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.
After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
