An employee was going to inspect a gas line for damage after the gas line was struck during excavation. The employee was looking into the excavation site when he fell 3 feet into the trench and sustained a left wrist laceration that required surgery and hospitalization.

An employee was helping to reposition a newly set pole that was being held by a digger derrick. While the winch line and choker chain were under tension, the employee started to rotate the pole. A link of the chain pinched the employee's left ring finger against the pole. The finger was fractured and the fingertip amputated, but was able to be reattached.

An employee was working on a part in the test facility when they made contact with an energized part; 7620 volts of electricity entered their body in the stomach area and exited through their hand, resulting in electrical burns. The employee was hospitalized.

An employee was assisting two co-workers in unloading a pallet jack from a pickup truck bed. The pallet jack began to fall and the employee reached to stop it when his hand went into the pallet jack and his fingertip was amputated by the force of the pallet jack falling down onto his finger.

An employee was standing next to a directional drilling machine when a plastic bracket from the drill struck the employee in the face causing three broken teeth and mouth lacerations.

An employee was unnesting tote bags when her grip slipped and the back of her left hand hit a box containing a steel step stool that was adjacent to the tote bags. The employee's hand was fractured.

An employee was cleaning up and putting away equipment when the second toe on her right foot was stubbed on a fixture, resulting in hospitalization and a subsequent amputation of the toe.

An employee was walking when he tripped on a concrete vault lid, jamming his big toe into the tip of his steel toe boots. The employee's big toe was medically amputated.

An employee was cleaning a bathroom. As the employee bent over, the corner of a sink hit the employee's right eyeball, causing an injury that required hospitalization and surgery.

On September 23, 2023, an employee was cleaning near a conveyor when they knelt with their right knee on the floor. Their knee contacted a small piece of glass on the floor, resulting in a laceration that required hospitalization.

An employee was unloading 10-foot sections of 6- to 8-inch pipe when their right ring finger was pinched between the pipe and pipe rack edge, resulting in a fingertip amputation at the first knuckle.

An employee was at a residential home to perform a gas houseline inspection after repairs. While using a flashlight to see, the employee walked into the laundry room and fell into a 2' x 2' access opening for a crawl space in the floor. A black rug was partially covering the opening. The employee fell approximately 40 inches into the crawl space. The employee sustained three fractured ribs on the left side, fluid on the left side of the lungs, and a bruised right knee.

Three employees were changing out a belt on a residual cooling fan when the injured employee's middle finger got caught between the belt and pulley. The employee sustained a fingertip amputation.

On November 11, 2024, an employee was offloading polyethylene pipe from a truck. The pipe was being moved along free-spinning rollers. The employee's pants got caught on a roller causing them to trip and fall onto a pipe. The employee sustained a puncture wound to their leg requiring hospitalization.

An employee was working on a compressed natural gas trailer gauge when a pressure release struck the employee's chest and forced the employee into a cement post. The employee's back was fractured.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
