On November 10, 2022, an employee had finished work in a telecommunications manhole. As he was lifting the front edge of the manhole cover onto the raised lip of its metal ring, his left middle finger became caught between the cover and the ring. The finger was partially amputated.

The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
