An employee was walking between a truck tailgate and a wellhead cellar to retrieve a well test chart. He slipped in the mud and fell eight feet into the cellar, resulting in a concussion, skull fracture, and forehead laceration.

Cactus Wellhead

An employee was standing on a ladder in a well cellar installing a tubing head with the assistance of a forklift operator. The tubing head was moved into place by running a 2-inch lifting strap through swivel hoist rings on opposite sides of the tubing head and around the forks of the forklift. The employee was removing the swivel hoist rings from the tubing head when the forklift operator started to boom in the forks of the forklift. This caused the strap, which was still being attached to the forks and the swivel hoist ring, to tighten and lift the swivel hoist ring housing. The employee's left thumb was caught and severely lacerated between the hoist ring housing and hoist ring cap screw.

Cactus Wellhead

An employee was conducting a pressure test inside a wellhead to check for leaks. When he removed his pressure detecting equipment, excess air pressure was released and struck the employee's hand and face. The employee suffered a facial laceration and a laceration and fracture to his hand. The employee was wearing PPE at the time.

Petroleum Service Corporation

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

Absolics Inc.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

American Residential Products, Inc.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

Berkheimer Maintenance

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

High-Tec Industrial Services, Inc.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

Renegade Group LLC

Employees were replacing a 24" pig trap door. A strap was shackled on the pig trap door and connected to a forklift. Welders began to cut the door off. An employee was holding onto the door when the cut was finished. The door shifted and caught his fingers, resulting in a hand injury and amputation.

Guidant Measurement

An employee was fitting an arbor fixture into a rotor housing to be reworked. The rotor is moved by a hoist. The arbor was being lowered into the rotor housing when the arbor slipped down onto his hand and amputated a portion of his left thumb.

Aztecwell

A crew was tripping out of the hole with an electrical submersible pump (ESP) in the process of swapping tubing tong heads to break down the ESP. An employee's left arm was caught and pinched by the tubing tongs. The employee sustained three lacerations and a fracture to the arm.

Hunting Energy Services

On May 21, 2025, at approximately 3:40 PM, a machinist was preparing a CNC lathe machine for it's next run. He was closing the machine door to complete the tooling setup when the door crushed his right middle finger. The employee required a surgical amputation to his fingertip.

Kisatchie Midnight Express

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
