An employee was preparing to change the pieces of steel used for the crusher rotor. The employee was under a bolt holding a 700-pound bar fell and crushed his left index finger against the rotor. He sustained a partial amputation of the fingertip.

An employee was on an elevated platform changing crusher screens. As he was standing in the middle of the guardrail, the screen pulled him over the side, causing the employee to fall 6-8 feet. He struck part of the crusher and the ground, resulting in two rib fractures.
