Final narrative
Employees were working on a drilling rig. They were putting bushings in with a hydraulic hoist when the cable became tangled in the derricks above. The hydraulic hoist cable came loose and threw the injured employee 15 feet into a piece of machinery used to screw together joints of drill pipe. The employee lost consciousness and sustained a head laceration that required staples and a knee laceration that required surgery.

An employee was pushing a stand of 4.5-inch drill pipe into position on the driller's side racking mat when he slipped and placed his hand down to catch himself. The stand was set down to floor level, landing on his left index finger. The finger was crushed, resulting in a partial amputation between the first and second knuckles.
A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway.
