An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was walking on gravel next to railroad tracks to locate underground utility lines when he slipped on loose rocks and rolled down the incline/slope. The employee was hospitalized with a possible neck injury.

An employee went into a building in a gated community. The employee was going up steps, then began walking backward down the steps, and fell. The employee was hospitalized with a fractured hip.

An employee was performing survey work in a field when he came into contact with a venomous animal. The employee sustained a puncture wound and swelling in his left hand and arm.

An employee was boarding a ship at anchorage via a pilot ladder to perform a survey on the ship. The employee fell from the ladder and landed back on the deck of the crew boat below. The employee sustained multiple fractures.

Employee 1 was setting up his survey instrument at the edge of a concrete apron surrounding an 11-foot-deep concrete utility vault with a 9x9 steel access cover/lid. Employee 2, then walked behind employee 1, stepping onto the steel vault access cover/lid. The cover gave way, causing employee 2 and the steel vault cover to fall into the vault approximately 11 feet below. Employee 1 fell 8 feet, striking an existing pipeline valve located within the vault. Employee 2 was hospitalized for fractures to the ribs on the left side. Employee 1 did not require hospitalization.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
