An employee was doing welding work on a cylinder. The cylinder fell from a skid frame. As the employee went to catch it, it partially amputated the bottom of his left middle finger.
An employee was servicing a track tongue scissor assembly that was hoisted. The equipment fell from the hoisting mechanism and onto their left little finger, resulting in an amputation.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
