An employee moved three pieces of material (weighing 290 pounds each) with a 5-ton overhead gantry crane, using a choker hitch. He then started the lowering process to place the load on top of a wooden dunnage board that was on the floor. His left hand was in between the material when the material buckled inward, crushing and amputating his left middle fingertip.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
