Two Men and a Truck

On November 28, 2022, an employee was descending the ramp of the truck when he slipped and fell three-to-four feet to the ground. The employee was hospitalized for a lower right leg fracture.

An employee was attacked while getting out of his car. An assailant hit him on the head twice with the butt of a gun, then shot him in the stomach after he turned around and wrestled for control of the gun. He was hospitalized.

An employee was moving a glass piece of furniture. The piece fell and lacerated the employee's right forearm.

The injured employee and a co-worker had just finished packing up items in a house and were transferring them to a new location when the injured employee felt chest pains as he was driving the moving truck. The employee had a stroke.

An employee was spotting for a truck driver who was backing up in the truck yard. The driver did not see the employee and pinned him between his truck and another truck nearby. He suffered a collapsed lung.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

An employee was moving a piece of furniture up a flight of stairs. At the top of the stairs, he lost his balance and fell backward onto a couch with a metal bracket sticking up out of it. He suffered a laceration to the abdomen and was hospitalized, requiring surgery.

An employee was standing on the hood of a box truck to fix a water leak in the cargo box of the truck. When he was descending the hood, he fell to the ground, resulting in a fractured pelvis.

An employee was unloading boxes from a truck at a residence, which included lifting boxes from the vehicle and placing them onto the ground to carry them into the residence. During this process, he suddenly experienced severe back pain, resulting in hospitalization due to a strain.

An employee was carrying a couch up the ramp of a moving truck with his partner. The employee was moving forward up the ramp, and his partner was moving backward up the ramp. The partner had reached the top of the ramp and was inside the truck while the employee was approaching the top of the ramp when a sudden gust of wind hit the employee and the couch. The employee fell forward and to the right, landing on his ribs at the top of the ramp. The employee suffered a fractured rib and a laceration to his liver that required hospitalization.

An employee was assisting a customer with attaching a trailer to their vehicle using a trailer mover. The trailer slipped from the trailer mover. As a result, the trailer mover struck the employee s left knee. The employee was hospitalized with a fractured left kneecap that required surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.
