An employee was preparing equipment when a pneumatic actuator fell and crushed his right middle finger, resulting in amputation of the fingertip.

An employee was cleaning the bacon department by spraying the floor and two running bacon conveyor belts with a hose and hot water. The employee placed her hand on the side of the conveyor to remove the slippery layer of bacon residue. Her left index and middle fingers contacted the conveyor belt next to a roller and was pulled into a structural part of the bacon press that had a metal edge. The employee sustained an amputation to her index and middle fingertips.

An employee was moving an incline conveyor in the sausage department when one of the wheels potentially hit a dip in the floor near a drain. The conveyor tipped over and landed on his left leg, fracturing the employee's femur.

An employee was spraying chemicals onto a propped up metal sheet on a wall when it fell over onto the employee's left foot, puncturing the toe guard and amputating his left little toe.

An employee was walking on a platform when he stepped on a roller. He lost his balance and fell, suffering hand fractures. He was hospitalized.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee was rinsing a food processing machine at a facility and went to stabilize himself. His left hand contacted the surface of the machine and a guard inadvertently closed, crushing the tip of his ring finger and resulting in an amputation and fracture.

An employee was performing janitorial duties on the 15th floor of a building that was under construction. She began having trouble breathing. The employee was hospitalized with lung irritation caused by dust.

On October 17, 2025, an employee was descending a fixed two-step stool when she slipped, landed on her left leg, and suffered a broken femur. She was hospitalized, requiring surgery.

An employee slipped while getting on a trailer. His chin struck the trailer jack, and he suffered fractures to his jawbone and thyroid cartilage. He was hospitalized, requiring surgery.

A crew was finishing work for the day at a job site. An insulated modular panel (IMP) being used as a door fell on an employee. She suffered a dislocated shoulder, a broken arm, and a gash on her left leg. She was hospitalized, requiring surgery for the arm/shoulder injury.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
