Final narrative
An employee was opening or closing the door of a trailer when wind caught the door, pushing it into the employee and causing them to fall to the ground. The employee sustained fractured hips.
An employee was unloading car parts from a trailer at a delivery site and fell from the liftgate to the ground. The employee sustained fractured ribs and a collapsed lung, as well as a head laceration from a car part that fell on his head.
An employee was manually closing a vehicle gate in a truck yard when their left middle finger became caught between the gate and the gate post. The fingertip was amputated.
On December 23, 2023, an employee was hooking air lines to their truck when they slipped on the step of a ladder and fell backward to the pavement below. The employee suffered a concussion.
An employee was using a fire extinguisher to put out a smoldering fire on a truck tailgate. The employee inhaled some of the smoke and fire extinguisher powder, suffered respiratory distress, and was hospitalized.

An employee was preparing to remove leaf spring bushings on the front drive axle of a tandem axle tractor. After raising the axle with a hydraulic jack, he placed two jack stands under the rear frame rails of the tractor and placed additional jack stands under the front drive axle. He then repositioned himself at the rear of the tractor where he reached for the release valve of the jack to begin lowering the axle (and frame) onto the jack stands. He was in a kneeling position while using his right hand to operate the release valve. His left hand was caught between the lowering frame rail and a jack stand. His left ring and middle fingertips were crushed, resulting in amputations.
An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.
An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.
On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
