An employee was preparing to move a large electrical box (switch gear unit) with an overhead bridge crane. As the switch gear box was lifted off the ground, the box shifted toward the employee crushing him against an air conditioning unit for the power control room. The employee sustained broken ribs and a lacerated kidney on the left side.

Powell Electrical Systems, Inc.

A temporary employee using a table saw to cut wood. The saw blade amputated his right pinky fingertip as he was adjusting the saw guide with a hammer. The blade was not guarded at the time.

An employee entered a freshwater, 3000-gallon tank to replace spray nozzles and was exposed to steam mist and residual chemicals that were left in the tank from the clean-out process. He experienced nausea from inhalation exposure and was hospitalized.

The overhead door came down and pushed the aerial lift over. The lift struck the concrete floor of the shipping bay and the employee sustained a compound fracture to his left forearm and a fractured pelvis.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
