An employee tried to sit on a tall, wheeled lab chair. She slipped off the seat, pushing the chair back and fell backwards against a lab refrigerator and hit her head. The employee was hospitalized for a head injury.

An employee was stepping out of a car when they slipped and fell on ice, resulting in a femur fracture.

An employee was closing up for the weekend when they slipped on oil and brake fluid and fell. The employee suffered a leg fracture.

An employee was working in the forest flagging an area to be harvested. He took a step and his foot slipped on a stick hidden under the leaves, causing him to fall to the ground. He landed on his right foot/lower leg resulting in a fractured tibia.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was walking down the hallway to get a trash bag from housekeeping when they slipped on the wet floor and fell. The employee sustained a fractured right knee.

On April 8, 2022, an employee was leaving the MRI Room and used her left hand to hold the door's handle while her right hand was holding the edge of the door. As the door closed, her right middle finger got caught between the door and the door jam resulting in amputation of her right middle fingertip without bone loss.

A maintenance employee was spreading ice melt outside of the building when the employee slipped and fell to the ground, fracturing three ribs.

An employee was taking cryogen readings from MRI equipment when he tripped over a cabling trough that was part of a new MRI staging set-up and impacted the floor. He suffered fractures to his left-side shoulder and hip.

An employee was working in the office when water entered the office as a result of a broken water pipe (exterior to the building). The employee was attempting to contain the water leak by spreading linens when she slipped and fell to the floor, breaking her right patella and requiring hospitalization.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
