An employee was unloading pallets of product from a trailer when a plastic-wrapped pallet became stuck against the bulkhead of the trailer. The employee was dislodging the pallet when he lost his grip and fell backward, striking his head on the side of the trailer. He then tripped over the forks of a pallet jack and fell to the floor of the trailer. The employee was hospitalized with a brain bleed and pain in his lower back and ribs.

An employee was cutting boxes with a safety knife. The safety guard on the knife broke off and the knife lacerated his left forearm. He was hospitalized.

An employee was on a catwalk five feet above the ground to see into a refrigerator trailer engine. As the employee was adjusting the belt on the engine, they fell off the catwalk to the concrete below, resulting in a left hip fracture. The employee was hospitalized.

On January 3, 2023, at 5:00 PM, an employee who had been using a powered industrial truck (PIT) was found lying on the concrete floor with lacerations and a broken nose requiring hospitalization.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.
