An employee had just finished repurposing chickens on a white tray that was sitting on the right side of a scale. He was moving the tray and stepping from the right to the left side of the scale when he twisted his left foot, heard a pop, and fell to the floor. He was hospitalized with a broken bone under his kneecap.

On June 10, 2018, an employee sustained an amputation to the right ring finger when using a meat cutting band saw.

An employee was selling memberships at a home and garden show. As she was walking to a vendor booth she tripped on the lip of a display riser and fell injuring her left knee and right arm.

On April 10, 2023, at 12:00 PM, an employee was using a broom to clear rock from a building's foundation in preparation for a spray-on vapor barrier. There was a 4-foot deep trench next to the building where the employee was working and the soil at the trench's edge fell away. The employee fell into the trench and was hospitalized for a left pelvis and/or femur fracture.

An employee was walking up a steel staircase to check wiring in the electrical room. The staircase had a top platform with a grate that was sitting across the beams of the platform. The grate had shifted and as the employee stepped on it, it gave way, causing him to fall approximately 3 feet to the ground below. The grate landed on the employee and he was hospitalized for four transverse process fractures in the lower spine.

An employee was working on a sump pump. The employee's right leg slipped into a pit full of boiling water and was burned.

An employee was cleaning a depositor machine to setup for a new product run. His right index finger was caught by moving parts in the depositor machine and the fingertip was partially amputated without bone loss.

An employee was under a label machine, working to remove a jammed label. The machine partially amputated her left index finger at the first knuckle.

An employee was operating an electric pallet jack. While standing beside the equipment, the pallet jack lurched forward, pinning the employee's leg against racking. The employee was hospitalized with a fractured right ankle.

An employee was on a top stock cart, moving items on shelving on the sales floor. The employee fell and suffered a broken left ankle. The employee was hospitalized, requiring surgery.

An employee was using a walkie stacker when the merchandise on it shifted and fell onto him. He suffered a head injury and a broken left leg that required surgery. He was hospitalized.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.
