A winch operator was preparing to load a chassis into a pipe (gel dispensing equipment) to connect the "mouse" attachment and pull jacket material over the mouse and fill nozzle. The rope assembly pulled the injured employee's hand into the pipe, pinching their hand between the end connector and the pipe nozzle. The employee sustained a puncture to the palm and an index finger fracture.

The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.
