An employee was standing on the rig, spooling the line, when the tubing line got caught around both ankles. The unit engaged and he sustained a left ankle fracture and right ankle sprain.

Employees were setting up pipe slips and the well-head ring needed to be centered. The injured employee was attempting to center the ring on the bottom of the flange. When the well flange came down, it caught his right pinky finger and smashed it at the joint. The finger was surgically amputated.
