On April 20, 2023, at 7:20 AM, an employee was removing a tire from a conveyor when their left ring finger was caught between the tire and conveyor belt. The employee's left ring fingertip was amputated.
On January 19, 2022, a press operator (employee 1) and a lift truck driver (employee 2) were performing die changeover duties. Employee 1 was attempting to remove a piece of scrap metal when his glove was caught in the quick change die plate. He lowered his left hand and employee 2 took it as a signal to move forward with the die on the lift truck. When the lift truck moved forward, employee 1's right index finger became caught in a pinch point between the die and the die stop, resulting in an amputation.
