On January 19, 2022, a press operator (employee 1) and a lift truck driver (employee 2) were performing die changeover duties. Employee 1 was attempting to remove a piece of scrap metal when his glove was caught in the quick change die plate. He lowered his left hand and employee 2 took it as a signal to move forward with the die on the lift truck. When the lift truck moved forward, employee 1's right index finger became caught in a pinch point between the die and the die stop, resulting in an amputation.

On December 18, 2020, an employee was struck by a forklift as it backed up. The employee suffered a compound fracture to the left leg and was hospitalized.

An employee was preparing to remove a plastics injection mold from a press. The employee's left ring finger was caught between a jam bar and a mold press plate and was amputated.

A press operator was helping to change out a press stamping die. The die was rolled from the press and onto a cart; the press operator's left ring finger was pinched between the die and the cart, sustaining a partial fingertip amputation.

On August 27, 2018, an employee was lifting a steel coil with a crane. The coil shifted and swung, and the employee's left little finger was pinched between it and the wall. The employee suffered a partial amputation.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

At 12:55 p.m. on October 8, 2025, a pneumatic cylinder press was activated while an employee's hand was resting on it. The moving shaft and top portion of the machine descended, catching the employee's right middle finger and amputating the fingertip (without bone loss).

An employee had been providing lawncare service and was driving back to the main facility when he experienced severe cramping due to heat stress. The employee was hospitalized for dehydration.

At about 5:15 p.m. on June 14, 2025, an employee was using a walk-behind mower to mow middle islands along a road. A support truck with an arrow board sign was following behind to control traffic. The truck lurched forward and struck the employee from behind, pinning the employee against the mower. The employee suffered a fractured hip, a right iliac wing fracture, right femoral nerve damage, and puncture wounds to the right leg. The employee was hospitalized.

A welder in the fabrication shop was straightening a wheel tab with a hammer and struck his left hand. The employee was hospitalized with a fractured hand.

A sprayer boom assembly fell off a sawhorse when the C-clamp was removed and it struck an employee. The employee suffered a separated collarbone and three broken ribs.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
