On April 17, 2017, an employee was hoisting material via crane and suffered a distal finger amputation after it was caught between the crane hook and hoist attachment.

On or about March 13, 2016, an employee's leg was crushed between a tugger unit and a stationary transformer unit, resulting in hospitalization.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was doing roof work. The employee fell from the roof to the ground about 12 feet below and suffered multiple fractures.

An employee was making copies at the copy machine. While walking away from the copy machine, her heel snagged on the carpet in the office and she fell. The employee sustained a fractured shoulder.

An employee was operating a vacuum truck, using the controls to raise the bed. As it rose, the bed touched an overhead power line. Electricity passed through the employee, entering through their left hand and exiting through their left toe.

An employee was walking down the hall when she lost balance and fell to the floor, resulting in a fractured keft hip.

An employee was loading equipment into a truck. An object struck the employee, who suffered a gash above the right eye and lost consciousness.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
