An employee was wearing cut-resistant gloves while operating an automatic slitter. The machine stalled and the employee placed their right hand on a panel to clean it before cutting. The slitter started running again, catching and damaging the cut-resistant glove. The employee's hand made contact with the slitting blades, resulting in amputation of the right ring finger and injury to two other fingers and lacerations to the hand. The employee was hospitalized. The machine was not guarded at the time.

An employee was carrying a bucket of water to the work bench when he tripped. His right foot became stuck between the base of the 28" mixer re-screener (machine was not in operation) and anchor bolts protruding from the floor. The employee fell with his foot stuck, resulting in a fractured ankle that required surgery and hospitalization.

An employee was carrying a 36-inch by 60-inch sheet of paper and stepped on the paper which caused him to trip and fall. The employee braced his fall with his elbow and sustained a fracture to the left humerus.

An employee was manually pushing an industrial cart/buggy by a container when his finger was caught between the cart and the container, resulting in an amputation of the left ring fingertip and nail.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was installing a core on a spindle that was on an extruder. The employee's left little fingertip became caught between the core and an air chuck, resulting in a fingertip amputation.

An employee was setting up a forming machine when they fell approximately 3-4 feet from an elevated work platform onto a fixed railing. The employee sustained a back injury.

An employee was performing a production process that includes rough cutting a part using a band saw. When the employee went to push the part into the saw blade, the blade cut the tip of his left thumb, resulting in an amputation.

On May 5, 2022, an employee was driving a tractor-trailer to the next job site and had a seizure due to heat exhaustion.

An employee was loading cardboard into a compactor. The compactor's safety door dropped onto his right hand, amputating the tip of his index finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
