An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

On September 22, 2025, at 11:45 a.m., an employee was conducting polishing activities on a lathe when the lathe contacted their right hand. The employee suffered amputations of the middle, ring, and little fingers at the middle knuckle and the loss of skin on the index finger, resulting in hospitalization and surgery.

An employee was assisting water treatment plant personnel with changing a flange when their finger got caught between the two pieces. The employee sustained a finger amputation.

An employee was delivering steel pipes to a customer. The customer was helping the employee readjust the steel pipes on a trailer using a forklift when the pipes rolled onto the employee's hand, resulting in two crushed and broken fingers.

Two employees were moving a vertical turbine pump into a shop using a set of jacks. One of the jacks hit a small hole in the concrete, causing the pump to jump and land on one employee's right foot. The foot was crushed and the employee was hospitalized.

An employee was lifting sheet metal from a welding table using a crane hook. The sheet metal slipped out and amputated the employee's left ring finger above the top knuckle.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
