An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

During a machine restring-up operation, the employee at the unwinder opened an access door in an elevated work platform to re-thread the machine and turned around. When he turned back, he stepped into the access opening and fell three feet. The employee's lower right leg was fractured.

An employee was installing a ramp weighing approximately 500 pounds from the sidewalk to the back of a trailer when his left ring finger became pinched between the ramp and the sidewalk, resulting in amputation.

A forklift driver got off a forklift to adjust a pallet. The top pallet of products fell, breaking the driver's leg. The driver was hospitalized and underwent surgery.

An employee was performing routine maintenance on a coating machine. A stuck idler roll sprung loose, jamming three of the employee's fingers between the idler roll and the frame of the machine. The employee's middle finger was injured.

An employee was pouring material into a vessel containing xylene and butanol when a flash fire occurred, burning the employee's face, ear, and neck.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
