An employee went to perform a routine visual inspection by opening the manway hatch of the tank. As he turned to the tank and opened the manway hatch, the steam and/or the caustic cleaning solution was released with a high force. The temperature of the tank holding the caustic solution was 200 F. The employee sustained chemical and thermal burns to his back and body. He also suffered a compound fracture to his right ankle and fractures to his right elbow and right scapula. The employee was hospitalized.

An employee was cleaning the hold containment kit area when he was exposed to a liquid, possibly sodium hydroxide. The employee sustained chemical burns to the right knee and shin and was hospitalized.

An employee's finger was caught in a set of double industrial automatic doors, causing an amputation of the right ring distal fingertip.

A forklift was backing out of a pick location after doing some work on a raw material container. While in reverse, the lift struck another employee in the back. This forced the employee's foot into an open end on a drum lifting device. The pressure compressed his steel toe boot and fractured toes on his left foot.

A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

A patient kicked a police officer in the leg, rupturing their patellar tendon. The police officer required hospitalization and surgery.

A cashier was hit in the face by a customer. The employee sustained injuries to his head and face.

An employee was making patient rounds at the hospital and stopped to talk with the charge nurse in front of the nurse's station. A patient reached over the nurse's station, grabbed a metal three-hole paper punch from over the desk, and proceeded to attack the employee with it. The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.

An employee was performing flagging operations on a closed road. A postal vehicle was allowed onto the road to make a delivery. The employee was struck by the postal vehicle as it was backing up, resulting in a broken leg.

An employee was walking and taking post-locate photos. He stepped on a ceramic drainage tile and his foot/leg punched through the tile. His leg was lacerated by the broken tile. The employee was hospitalized with internal bleeding due to a punctured femoral artery.

Employees were preparing to place a section of metal grating measuring approximately 50" x 18" and weighing 40 to 50 lbs. The injured employee grabbed the grating to pull it toward himself and another employee was moving to the other side to assist with the lift. As the injured employee pulled, his right foot slipped and twisted. He was hospitalized with a fractured right leg.

An employee was in their personal vehicle awaiting instructions from the construction contractor on posting traffic cones. The driver of a motor vehicle swerved to avoid a steel plate in the roadway and struck the employee's car. The employee was hospitalized with four fractured ribs.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
