An employee was operating a machine that prints logos onto fabric materials when the machine began to leak ink. As the employee was attempting to remove ink from the roller using a towel, the rollers caught the towel and pulled her hand into the machine. Her right little finger was crushed and de-gloved. Her finger had to be surgically amputated.

Two employees were working on a scissor lift approximately 15-20 feet above the ground, positioned beside a larger production line with a moving trolley. As the trolly retracted, its external motor struck the basket of the lift, causing the lift to tip over. The injured employee and the lift fell to the ground and they suffered a laceration to the right knee that required hospitalization.

An employee was working in a storage area consolidating pallets of yarn when his shoelace got caught on the corner of a pallet. The employee lost his balance and fell to the floor, resulting in a right femur fracture in the hip area.
