An employee was pre-rigging a well pad and torquing down the monoline. His hands were placed on the head of the hydraulic torque wrench while moving it to a different bolt. At the same time, the employee's foot slipped and the torque wrench activated. His left little finger went between the nut and head, resulting in a fingertip amputation.

A mechanic was spotting for a crane that was lowering an engine onto a frame. As the employee cleared the threads of the engine mount, the weight of the engine shifted, amputating his left index fingertip before the first knuckle.

An employee was run over by a telehandler. The employee suffered a broken pelvis and was hospitalized.

An employee opened a generator cap and received third degree burns requiring hospitalization.

A mechanic was on top of the HHP in Bay 1 drilling out broken bolts. Another operator was running the overhead crane using a handheld remote, moving it from the south side of the HHP to the north side to pick up a fluid end. As the crane moved over the top of the HHP, it struck the mechanic, knocking him off the HHP onto the cement floor of the shop 13 feet below. The injured employee was hospitalized for broken ribs, a sprained wrist and stitches to the face.

An employee was power washing a metal tank from an elevated platform. They fell through a gap between the platform and the tank. The employee landed on the ground and sustained fractures to their ankles and feet.

An employee was installing a metal roof when he stepped on a skylight. The employee fell through the skylight to the ground, resulting in head and shoulder fractures.

An employee was replacing an HVAC unit in a residential home. While he was in the attic space above the garage, the wood he was walking on broke, causing the employee to fall through the sheetrock to the concrete floor below. The employee bumped his head and sustained a shoulder injury as well as a broken ankle.

An employee was putting a tarp on a water tank while standing on a trailer. She lost her footing and fell through the trailer bed, striking the corner of the trailer bed. The employee sustained internal bleeding.

An employee was repairing a sub-floor in a residential house that had water and fire damage. The employee fell through the sub-floor and landed in the basement, resulting in spinal fractures that required hospitalization.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.

An employee was using a torch to loosen a bolt on a water trailer. Natural gas was present in the water and it ignited, causing a fire. The employee sustained third-degree burns to their upper leg.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
