An employee was walking to retrieve a forklift when they fell through a floor opening into a sump pit. The employee sustained chemical burns from their knees to their feet from the ammonium phosphate in the pit.

An employee was pre-rigging a well pad and torquing down the monoline. His hands were placed on the head of the hydraulic torque wrench while moving it to a different bolt. At the same time, the employee's foot slipped and the torque wrench activated. His left little finger went between the nut and head, resulting in a fingertip amputation.

A mechanic was spotting for a crane that was lowering an engine onto a frame. As the employee cleared the threads of the engine mount, the weight of the engine shifted, amputating his left index fingertip before the first knuckle.

As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
