An employee was inside of a permanent filter media pod hydroblasting the filter media with an 8,000 psi wand. The employee lost their balance and the water from the wand made contact with their foot, resulting in a puncture wound.
While hooking up a pump trailer to the hitch on a truck, an employee's finger was caught between the hitch ball on the truck and the coupler on the trailer. The employee sustained a crushing injury/fracture to the left middle finger that required surgical amputation.
